Patient No-Show Policy

On the occasion of the first “No-Show” appointment, the patient will be invoiced the appropriate amount of the missed appointment. No further appointments will be booked until the invoice has been paid in full.

  • Future appointments will not be booked until all invoices owing are paid in full.

    We apologize for those occasions that the physician is behind schedule – we hope to keep these to an absolute minimum.

    Cancellation Policy

    Please cancel your appointments 24 business hours in advance so that the time allotted can be given to others. For cancellation of Monday appointments, call the clinic at 403-444-0428 on Friday during our business hours. All Late Cancellations will be invoiced the appropriate amount of the appointment. No further appointments will be booked until the invoice has been paid in full.

    Abuse of Staff

    Abuse of staff either over the phone, in writing, online, or in person will not be tolerated and will be grounds for immediate termination.

    Designated Physician

    Once a patient has been accepted into one of the Physician’s Practices, that patient will not be able, at any point, to transfer to another physician within the clinic. When appointments/visits are required that patient will be placed with his/her physician or his/her designate unless either are not available. If you are a personal friend of one of the physicians, it is the policy of the clinic that your designated physician be one of the other physicians in the clinic.

    Uninsured Services

    The Alberta Health care Insurance Plan pays for many of the services patients receive from their doctor. However, a number of services are not covered by the government plan. These are uninsured services. Some are billed to lawyers, government agencies or others. Some are billed to the patient.

    Examples of uninsured services include: Telephone Prescription Refills, Advice by phone, Transfer of records, Private letters/forms (eg. Sick Notes), Insurance forms (eg. Disability), Blue Cross Special Authorization Forms, Third Party Letters/examination, Pregnancy Leave and Employment Insurance forms, Certification of fitness, Disabled Parking Authorization, Driver’s Medical, Certain Uninsured Procedures, Missed Appointment Fee, & Costs of Uninsured Supplies.

    Requirements for Prescription Renewal by Phone or Fax:

    All prescription refill requests may or may not be approved at the discretion of the Chinook Mall Medical Clinic Physician.

    • A controlled substance or antibiotic will not be refilled in this manner. These medications require an appointment.
    • The specific pharmacy of choice, with phone and fax numbers is provided by the patient.
    • The medication has been previously prescribed by a Chinook Mall Medical Clinic Physician.
